How to use

Follow directions on the product label, information guide, and provided by your doctor before using Hydrochlorothiazide+Valsartan. Eat this medicine as per your doctor's instructions.
Hydrochlorothiazide+Valsartan is eaten with or without food. Hydrochlorothiazide+Valsartan is eaten This medicine should be taken usually in the morning.

Typical Dosage

The typical adult dose of Hydrochlorothiazide+Valsartan is 160 mg valsartan and 12.5 mg hydrochlorothiazide to 320 mg valsartan and 25 mg hydrochlorothiazide once a day. The maximum dose for adults of Hydrochlorothiazide+Valsartan is 320 mg valsartan and 25 mg hydrochlorothiazide per day. Hydrochlorothiazide+Valsartan is typically used at the same time every day.

Tell your doctor if you develop new symptoms. Talk to your doctor if dry mouth, drowsiness, muscle fatigue, thirst, restlessness, very low urine output, lack of energy, confusion, fast heartbeat, weakness, seizures, nausea, vomiting, muscle pain, cramps, skin rash, a decrease in vision, and eye pain. If you have issues with the health of your liver, A low starting dose is recommended in patients with liver dysfunction. Consult with your doctor before stopping the use of Hydrochlorothiazide/Valsartan.
To see the impact of Hydrochlorothiazide/Valsartan on the body, your doctor may recommend a lower initial dose. Please follow your doctor's instructions carefully. Older patients may see an increase in the risk of side-effects. Hence, a lower dose may be recommended.

Lab Tests

Before starting this medicine, your doctor may ask you to undergo certain medical or laboratory tests (Sodium) to monitor for side-effects. After starting the medicine, your doctor should monitor you for these tests to make sure that you do not have side-effects of this medicine. Your doctor may require that certain tests be performed before using Hydrochlorothiazide/Valsartan. The doctor may ask you to have Sodium test. Storage

Store Hydrochlorothiazide/Valsartan at 15-30°C (59-86°F), and protect from moisture and keep it in an air-tight container. Keep this medicine away from children and pets.

Special Instructions

Mild to Moderate Liver Dysfunction
In the patients with mild to moderate liver dysfunction without reduction or stoppage of bile flow (cholestasis), the maximum dose of valsartan should not be greater than 80 mg.

Missed Dose

Take the missed dose as soon as you remember. Skip the missed one if it's time for next dose and follow your regular dosing schedule.

Overdose

What to do if you overdose on Hydrochlorothiazide+Valsartan?
The treatment of overdose depends upon the ingestion time, type and severity of the symptoms of overdose. In case of low blood pressure, the patient should be placed in a lying position, and salts supplements should be given as soon as possible. Hydrochlorothiazide can be cleared from the body by using an artificial blood purifying technique (dialysis), but valsartan cannot be eliminated from the body through this process.

Precautions while using Hydrochlorothiazide+Valsartan

Before you use Hydrochlorothiazide+Valsartan, tell your doctor of your medical and health history including the following:
  • allergy
  • bronchial asthma
  • diabetes
  • disturbed functioning of the kidney
  • high calcium level in the blood
  • high uric acid level in the blood (gout)
  • low potassium levels in the blood

The patients with a history of allergy or bronchial asthma are more likely to have severe allergic reactions to hydrochlorothiazide.

The use of Hydrochlorothiazide/Valsartan may change blood bilirubin level. Hydrochlorothiazide+Valsartan may increase the level of bilirubin in the blood.
The use of this medicine may change hemoglobin and hematocrit value. Hydrochlorothiazide+Valsartan may decrease the level of hemoglobin and the volume of red blood cells in the blood (hematocrit).
The use of Hydrochlorothiazide/Valsartan may change creatinine and blood urea nitrogen (BUN). Hydrochlorothiazide+Valsartan may raise the level of waste products in the blood (creatinine and BUN (blood urea nitrogen)).
The use of this medicine may change sodium level. Hydrochlorothiazide+Valsartan may decrease the blood sodium level.
The use of this medicine may change potassium level. Hydrochlorothiazide+Valsartan may decrease the blood potassium level.

Use when Pregnant

Hydrochlorothiazide+Valsartan is not safe for use in women who are pregnant. If you are pregnant or planning to become pregnant, you should discuss the impact on the fetus with your doctor before you start using this medicine. Use of Hydrochlorothiazide+Valsartan is not recommended in pregnancy as it may cause injuries to the fetus (decreased functioning of the kidney, deficiency of amniotic fluid, poor development of skull) and kidney failure.

If Breastfeeding

Hydrochlorothiazide/Valsartan is not safe for use in women who are breastfeeding. If you are breastfeeding, discuss with your doctor if you should either discontinue breastfeeding, or stop using this medicine during this period. Hydrochlorothiazide may pass into human milk.

If trying to Conceive

Consult with your doctor on the use of Hydrochlorothiazide+Valsartan, if you are trying to conceive.

Alcohol

Avoid drinking alcohol with Hydrochlorothiazide+Valsartan. Consumption of alcohol may cause low blood pressure, dizziness, and fainting.

Seizures

Hydrochlorothiazide+Valsartan can make you feel sleepy. Be careful, especially while driving, while using heavy machinery, or when doing any activity that needs you to be completely alert. The consumption of alcohol with Hydrochlorothiazide+Valsartan can worsen the sleepiness. Hydrochlorothiazide+Valsartan may cause seizures in some people. Hence, you should discuss with your doctor before performing any activities where a loss of consciousness may cause harm to you or others.

Increase in Risk

This medicine may increase your sensitivity to sunlight and make you prone to sunburn. If this happens, limit your time outdoors to prevent sunburn. Use a sunscreen and cover your skin.

  • Infections

  • Kidney disorders
    Symptoms: kidney failure, kidney dysfunction, swelling in between the kidney tubules (interstitial nephritis) kidney dysfunction
    If progressive kidney dysfunction becomes clear, consider withholding or discontinuation of diuretic therapy.
  • Severe skin reactions
    Symptoms: severe skin reactions, peeling and scaling of the skin (exfoliative dermatitis), severe allergic reactions, sun allergy, inflammation of blood vessel walls (necrotizing angiitis), fever swelling of the area under the skin
    If this happens, stop using this medicine. If retreatment with diuretics is necessary, it is advised to avoid exposure of skin to the sun and artificial ultraviolet rays.
  • Digestive system disorders
    Symptoms: inflammation of the pancreas (pancreatitis), yellowing of the skin (jaundice) loss of appetite (anorexia)
  • Symptoms: body stops producing enough new blood cells (aplastic anemia), decreased white blood cell count, decrease in the number of leukocytes, abnormal breakdown of red blood cells, low level of thrombocytes, excessive fluid in the lungs (respiratory distress), inflammation of lung tissue (pneumonitis), swelling in the arms, legs allergic reactions
  • Symptoms: high blood sugar levels excess of sugar in the urine
  • Vascular disorders
    The patient should use caution for the symptoms of low blood pressure. If this happens, discontinue the drug and consult with your doctor.
  • Heart disorders
    Symptoms: increased heartbeat

Warnings

Abnormal Functioning of the Liver

Patient with the condition of the abnormal liver function or other liver diseases is at increased risk when using this medicine. The patient using Hydrochlorothiazide+Valsartan therapy can develop changes in the blood and electrolyte balance that may lead to liver damage. The patient should use the drug with caution.

Pregnancy

The patients who are pregnant or who are planning to conceive are at increased risk when using this medicine. The patient should discontinue this Hydrochlorothiazide+Valsartan therapy.

Lithium

Patients using lithium during thiazide therapy are at an increased risk while using this medicine. Use of diuretic agents (Hydrochlorothiazide+Valsartan) reduce the clearance of lithium by the kidney which may lead to fatal conditions. The lithium should not be given during the thiazide therapy.

Fluid or Electrolyte Disturbances

The combined use of Hydrochlorothiazide+Valsartan with potassium supplements, potassium-sparing diuretics, salt substitutes containing potassium, or other agents may increase blood potassium levels. The patient should be monitored periodically for blood electrolytes, and Hydrochlorothiazide+Valsartan therapy should be discontinued in the case of evident kidney damaging.

Abnormal Functioning of the Kidney

Patients with the condition of heart failure, narrowing of arteries that carry blood to the kidneys or having treatment with angiotensin converting enzymes and angiotensin II receptor blockers are at a higher risk when using this medicine. Patient taking Valsartan therapy has been associated with decreased production of urine and abnormally high levels of nitrogen in the body with abnormal functioning of the kidney and it can be fatal. But in the case of ACE inhibitors, there is an increase in creatinine level and blood urea nitrogen. Patient should use the drug with caution and there should be periodic monitoring of potassium and creatinine levels while taking Hydrochlorothiazide+Valsartan therapy.

Nearsightedness and Increased Intraocular Pressure of the Eye

Patients with the known history of sulfonamide or penicillin allergy are at an increased risk when using this medicine. The combined use of Hydrochlorothiazide and sulfonamide can produce drug reaction which results in nearsightedness, increased intraocular pressure of the eye, decreased visual clarity, pain in the eye and if it is not treated loss of vision can occur. The patient should discontinue the use of hydrochlorothiazide and surgical treatment should be provided when the situation is uncontrolled.

Increased Sensitivity to Sunlight

Patient taking thiazide diuretics have been reported with the allergic reaction of sunlight. The patient should be advised to stop the Hydrochlorothiazide+Valsartan treatment and if reuse is required then protect visible areas from exposure of sunlight.

Newborn Disease and Their Fatality Rate

Patients who are pregnant or who want to conceive are at the higher risk when using this medicine. Studies suggest that ACE inhibitor including Hydrochlorothiazide+Valsartan therapy is associated with injury in newborns such as high blood pressure, poor skull development, prevents or limits the normal excretion of urine, reversible or irreversible kidney damage and fatality. Intrauterine exposure to the use of thiazide diuretics within the uterus cause yellowing of skin of the newborn, decrease in platelets and other side effects. The patient should be informed of the possible risk to the fetus.

Low Blood Pressure in Salt Depleted Patients

Patient with a condition of abnormal blood pressure or in the case of a decrease in extracellular fluid volume when salt and fluid declines are at increased risk when using this medicine. Patient receiving large doses of diuretics may have an excessive reduction of blood pressure. This dose should be adjusted before the use of Hydrochlorothiazide+Valsartan and therapy should be started under the proper medical guidance.

Abnormal Liver Function

Patient with the condition of abnormal liver function or with progressive liver disease and in case of a blockage in the duct that takes bile from the liver to the gallbladder are at an increased risk when using this medicine. The patient using hydrochlorothiazide may accelerate brain damage because of liver failure and in the case of valsartan, it showed lowered clearance of valsartan. The patient should use hydrochlorothiazide with caution and care should be taken while taking valsartan.

Allergic Reactions

The patients with or without the condition of allergy or breathing disorder are at a higher risk when they are using this medicine. The patient using hydrochlorothiazide may have an allergic reaction.

Corticosteroids

Your doctor's guidelines may need to be followed while taking this medicine along with corticosteroids, which are used to treat allergic conditions. There have been seen increased electrolyte deficiency, particularly low potassium level.

Antidiabetic Drugs

Hydrochlorothiazide/Valsartan interacts with antidiabetic drugs (metformin), which are used to lower the blood glucose level. Use of metformin with hydrochlorothiazide may lead to building up of acids in the body. Proper dose adjustment of antidiabetic medicine is required in this case.

Antibacterials, Psychoactive Agents, and CNS Depressants

There may be an interaction of Hydrochlorothiazide+Valsartan with alcohol, which helps in depressing the activities of brain or barbiturates, which is used in the treatment of insomnia or narcotics, which are effective at reducing pain. Potentiation of low blood pressure that happens after standing up from sitting or lying down may occur. The patient should be cautious while taking these medicines with Hydrochlorothiazide+Valsartan.

Angiotensin II Receptor Antagonist

Hydrochlorothiazide/Valsartan may interact with angiotensin II receptor antagonist including valsartan, which is used to treat high blood pressure and heart failure. The combined use of inhibitors of the efflux transporter (ritonavir) or uptake transporter (rifampin, cyclosporine) may increase the absorption of valsartan. Patients who are elderly, have reduced fluid volume, or with improper functioning of kidneys, use of NSAIDs such as selective COX-2 inhibitors, with valsartan, may result in a decline of the functioning of kidneys, including possible acute kidney failure. Patients receiving NSAID therapy and valsartan should be monitored of the functioning of kidneys regularly. Proper care should be done when starting or stopping combined treatment with these drugs.

Bile Acid Sequestrants

Special instructions need to be followed while taking this medicine along with bile acid sequestrants (cholestyramine and colestipol), which are used to lower cholesterol levels in the body. Use of cholestyramine or colestipol with Hydrochlorothiazide+Valsartan may decrease the absorption of hydrochlorothiazide. It is advisable to use hydrochlorothiazide at least 4 hours before or 4-6 hours after the use of resins (cholestyramine or colestipol), which would potentially minimize the interaction.

Laboratory testing

Your doctor's guidelines may need to be followed while taking this medicine along with laboratory testing. Occasional elevations of levels of proteins during liver function test occurred in Hydrochlorothiazide+Valsartan-treated patients. Abnormally low count of neutrophils was seen in some of the patients treated with Hydrochlorothiazide+Valsartan. The decrease in hemoglobin and low red blood were examined in patients receiving Hydrochlorothiazide+Valsartan. Minor rises in creatinine and Blood Urea Nitrogen occurred in patients taking Hydrochlorothiazide+Valsartan.

Nonsteroidal Anti-inflammatory Drugs

Hydrochlorothiazide/Valsartan interacts with Nonsteroidal anti-inflammatory drugs, which are used to reduce pain, decrease fever, prevent blood clot formation. Use of NSAIDs with Hydrochlorothiazide+Valsartan may lead to the worsening of the functioning of the kidney and increase in the blood potassium levels. It is recommended to monitor the functioning of the kidney as well as hydrate the patient.

Antiepileptic Drugs

Special instructions need to be followed while taking this medicine along with carbamazepine, which are used to prevent and control seizures. The combined use of Hydrochlorothiazide+Valsartan and carbamazepine may lead to symptomatic low sodium level. It is recommended to be cautious in long-term use of these drugs.

Lithium

There may be an interaction of Hydrochlorothiazide+Valsartan with lithium, which is the treatment of major depressive disorder. Use of diuretic agents (Hydrochlorothiazide+Valsartan) reduce the clearance of lithium by the kidney which may lead to fatal conditions. Lithium should not be given with diuretics. Patients should refer to the package insert for lithium preparations before the use of Hydrochlorothiazide+Valsartan.

Nondepolarizing Skeletal Muscle Relaxants

Hydrochlorothiazide/Valsartan may interact with nondepolarizing skeletal muscle relaxants (tubocurarine), which helps to decrease the muscle tone. There is possibly increased quick positive reaction to the muscle relaxant. The patient should be cautious while taking both drugs in combination.

Dual blockade of Renin-Angiotensin-System

Hydrochlorothiazide/Valsartan may interact with angiotensin II receptor blockers (ARBs) and angiotensin-converting enzyme inhibitors (ACEIs), or aliskiren which are used for the treatment of high blood pressure. The combined use of ACE-inhibitors, angiotensin II receptor blockers or aliskiren increased the risk of low blood pressure, high levels of potassium in the blood and decreased functioning of the kidney (including acute kidney failure). The use of Hydrochlorothiazide+Valsartan with these agents is not recommended.

Antiviral Agents

Your doctor's guidelines may need to be followed while taking this medicine along with amantadine, which is used to treat influenza A viral infection. Hydrochlorothiazide may raise the risk of side-effects caused by amantadine. Patients should take necessary precautions while taking this combination therapy.

Antihypertensives

Special instructions need to be followed while taking this medicine along with antihypertensives (methyldopa), which is used to reduce the high blood pressure. Use of methyldopa with hydrochlorothiazide may lead to the condition of the abnormal breakdown of red blood cells. Patients should be cautious while taking the combination therapy of methyldopa with hydrochlorothiazide.

Digitalis glycosides

Hydrochlorothiazide/Valsartan interacts with cardiac glycosides (digitalis glycosides), which are used to treat heart failure and irregular heartbeat. Thiazide-induced low potassium level or low magnesium level may occur as undesired effects promoting the onset of digitalis-induced irregular heartbeat. While taking a combination of both drugs patients should be cautious.

Calcium Supplements

There may be an interaction of Hydrochlorothiazide+Valsartan with calcium salts, which are used to fulfill the calcium deficiency. Use of thiazide diuretics, including hydrochlorothiazide, with vitamin D or with calcium salts may increase the levels of calcium in the blood. Patients should be cautious while taking the combination therapy.

Beta Blockers

Hydrochlorothiazide/Valsartan may interact with beta-blockers, which are used to treat high blood pressure and migraine. The combined use of hydrochlorothiazide, with beta blockers, may increase the risk of high blood sugar levels. Patients should take necessary precautions while using Beta Blockers with thiazide diuretics.

Uricosuric Drugs

There may be an interaction of Hydrochlorothiazide+Valsartan with probenecid, sulfinpyrazone, and allopurinol, which are used in the treatment of gout. Hydrochlorothiazide may increase the level of blood uric acid. The combined use of thiazide diuretics, such as hydrochlorothiazide, may increase the incidence of allergic reactions to allopurinol. It is necessary to adjust the dose of medicines reducing the levels of uric acid in blood.

Potassium-sparing diuretics and potassium supplements

Hydrochlorothiazide/Valsartan interacts with potassium supplements, potassium-sparing diuretics, salt substitutes containing potassium, increases blood potassium levels. If these medicinal products which may affect potassium levels are considered important in combination with valsartan, proper monitoring of potassium blood levels are recommended. This combination should not be advised to the patients.

Antiarrhythmics and Antipsychotics

Special instructions need to be followed while taking this medicine along with antiarrhythmics, which are used to suppress abnormal rhythms of the heart and antipsychotics, which are used to treat serious mental health conditions. There is a risk of low potassium level, when hydrochlorothiazide is used with drugs that could induce torsades de pointes, particularly, antiarrhythmics and some antipsychotics. Take necessary precautions while using hydrochlorothiazide with antiarrhythmics and some antipsychotics.

Anticholinergic Agents

Your doctor's guidelines may need to be followed while taking this medicine along with anticholinergic agents (atropine and biperiden), which helps to reduce the contraction of smooth muscles. The absorption of thiazide-type diuretics may be enhanced by anticholinergic agents, possibly due to a decrease in the stomach and intestinal motility. While using thiazide-type diuretics with anticholinergic agents, the patients should take necessary precautions.

Interactions of Hydrochlorothiazide+Valsartan by Severity

The following medicines may interact when taken together and can increase your risk of harmful effects. Please consult with your doctor or pharmacist before taking these medicines together.

When should Hydrochlorothiazide+Valsartan be not used?

Allergic Reaction

Hydrochlorothiazide+Valsartan should not be used in patients who are allergic to any component of the Hydrochlorothiazide+Valsartan product. Due to the hydrochlorothiazide component, Hydrochlorothiazide+Valsartan should not be used in patients with failure of the kidneys to produce urine or allergic reaction to other sulfonamide-derived drugs.

Pregnant Women

Pregnant women with second and third-trimester pregnancy should not use Hydrochlorothiazide+Valsartan.

Severe Liver Abnormalities

Patients with severe liver damage, reduction or stoppage of bile flow and damaged bile ducts in the liver should not use Hydrochlorothiazide+Valsartan.

Severe Kidney Damage

Hydrochlorothiazide+Valsartan should not be used in patients with severe kidney damage.

Electrolyte Imbalance and High Uric Acid

Patients with low potassium level, high calcium level, low sodium level, and an excessive amount of uric acid should not use Hydrochlorothiazide+Valsartan.

Use of Hydrochlorothiazide+Valsartan with Aliskiren-Containing Products

The combined use of Hydrochlorothiazide+Valsartan with aliskiren-containing products should not be used in patients with high blood sugar level or kidney damage.

Patients with Gout

Patients with severe pain, redness and tenderness in joints (gout) should not use Hydrochlorothiazide+Valsartan.

Traveling With Medication

  • Ensure that you carry enough doses of each of your prescription medicines to last the entire trip. The best place to store your medicines is in the carry on baggage. However, while flying, if carrying liquid medicines, make sure you do not go over the limits imposed for carry-on liquids.
  • While traveling overseas, make sure that you can carry each of your prescription medicines legally to your destination country. One way to ensure this is by checking with your destination country's embassy or website.
  • Make sure that you carry each of your medicines in their original packaging, which should typically include your name and address, and the details of the prescribing doctor.
  • If your travel involves crossing time zones, and you are required to take your medicine as per a fixed schedule, make sure that you adjust for the change in time.

Expired Medication

Taking a single dose of expired Hydrochlorothiazide+Valsartan is unlikely to cause a side-effect. However, please discuss with your doctor or pharmacist, if you feel unwell or sick. An expired medicine may become ineffective in treating your prescribed conditions. To be on the safe side, it is important not to use an expired drug. You are much safer by always keeping a fresh supply of unexpired medicines.

Safe Disposal of Medication

  • If there are disposal instructions on the package, please follow the instructions.
  • If there are medicine take-back programs in your country, you should contact the respective authority to arrange for the disposal of the medicine. For example, in the USA, the Drug Enforcement Administration regularly hosts National Prescription Drug Take-Back events.
  • If there are no take-back programs, mix the medicine with dirt and place them in a sealed plastic bag. Throw the plastic bag in your household trash. Separately, remove all personal information including the prescription label from the medicine packaging and then dispose off the container.
  • If specifically indicated on the medicine package that it needs to be flushed down the toilet when no longer needed, perform the required step.
