Why it's used

Opime Tablet is used to treat high blood pressure. It is a prescription medicine. This medicine lowers the blood pressure by relaxing the blood vessels and reducing the force required to maintain blood flow from the heart. Opime is also used to treat long-term chest pain called as Angina. This medicine helps to control chest pain by increasing the amount of blood supply to the heart. It is also used to treat chest pain at rest (variant angina). Opime is also used to reduce the effects of damage in the heart blood vessels called as Coronary Artery Disease.
Opime Tablet may also be used along with other medicines in the treatment of certain conditions as recommended by the doctor. It is used with antianginal agents to treat long-term chest pain or chest pain at rest. It is also used together with other Anti-Hypertensive agents to treat high blood pressure.
Calcium Channel Blockers
Opime belongs to the Calcium Channel Blockers class of medicines. Calcium channel blockers treat a variety of conditions, such as high blood pressure, chest pain and symptoms of cold hands and feet (Raynaud's disease). These medicines relax blood vessels and thereby increase the supply of blood and oxygen to the heart.

How to use

Read the directions on the product label, patient guide, or medicine guide provided by the medicine company or your pharmacist before starting to use Opime Tablet. If you have any questions related to this medicine, ask your doctor or pharmacist. Consume this medicine as recommended by your doctor.
Opime Tablet is consumed with or without food. Avoid more than one dose of Opime at a time. Opime Tablet is consumed usually with breakfast, dinner or at bedtime.

Typical Dosage

The typical dose of Opime is 5-10 mg once daily. The usual dose for children is 2.5-5 mg once daily. The maximum adult dose of Opime is 10 mg in a day. Opime is commonly used at the same time every day. This makes is easier for patients to not miss a dose of this medicine.
This medicine is to be used for longer periods of time. This medicine is prescribed for long-term use in patients with angina. You should continue to use this medicine as directed by the doctor even if you feel well.
If you have issues with the health of your liver, an initial dose of 2.5 mg once daily is prescribed in patients with abnormal functioning of the liver. You should consult with your doctor before stopping the use of Opime.
Your doctor may recommend a lower initial dose of this medicine to see the impact of this medicine on the body. Please follow your doctor's directions.

Use in Children

Possibly safe with Alcohol

Storage

You should store Opime Tablet at room temperature 15°C-30°C (between 59°F and 86°F), away from moisture, and away from light. Store the medicine away from the reach of children and pets.
Medicines may be prescribed for uses other than those listed in the medicine guide. Do not use Opime Tablet for conditions for which it was not prescribed. Do not give Opime Tablet to other people who might have the same conditions or symptoms that you have. Self-medication may harm them.

How to take Opime

Your dose and how often you take Opime will depend on the following factors:

Special Instructions

Oral solution
This medicine should be given via insertion of the tube in the nose (nasogastric tube) and stomach (percutaneous endoscopic gastrostomy tube).

Missed Dose

A missed dose should be taken as soon as you remember it. However, if the time for the next dose is almost there, then the missed dose should be skipped, and the regular dosing schedule should be continued. Avoid taking a repeated dose to make up for a missed one.

Overdose

What to do if you overdose on Opime?
During an overdose of Opime initiate careful monitoring of respiratory and heart parameters. In case if low blood pressure remains unresponsive to these conservative measures, consider intake of vasopressors (such as phenylephrine). In some cases of moderate intoxication, vomiting, cleaning out the contents of the stomach (gastric lavage) helps to reduce the harmful effects of this medicine. In healthy individuals, absorption of Opime can be reduced by taking charcoal up to 2 hours after the intake of 10 mg Opime. Charcoal is a form of carbon that has small, low-volume pores. These pores help trap chemicals as in the case of poisoning. Calcium gluconate, a mineral supplement intravenously can prove to be helpful in reversing the effects of calcium channel blockade.

Precautions while using Opime

Before you use Opime, tell your doctor of your medical and health history including the following:

Dizziness

Increased Risk

This medicine may increase your sensitivity to sunlight. If this happens, limit your time outdoors and prevent sunburns. Cover your skin or use a sunscreen.

Warnings

Elderly Patients

These patients are at an increased risk when using this medicine. While increasing the recommended dose in such patients, proper care should be followed.

Chest Pain and Heart Attack

Patients with increased chest pain and heart attack are at increased risk when using this medicine. While initiating or increasing the dose of Opime may worsen the condition of chest pain and heart attack in patients with severe heart diseases.

Narrowing of the Aortic Valve Opening

This medicine may cause low blood pressure in such patients.

Abnormal Functioning of the Liver

The levels of Opime get increased in patients with liver failure. Careful monitoring and a low dose of Opime is required in patients with abnormal functioning of the liver.

Heart Failure

Patients with heart failure are at an increased risk when using this medicine. Such patients may see an increased risk of heart problems and fluid buildup in the lungs. Take necessary precautions while using this medicine in patients with a history of heart failure.

Interactions with Opime

When two or more medicines are taken together, it can change how the medicines work and increase the risk of side-effects. In medical terms, this is called as a Drug Interaction.

‎Lipid-Lowering Agents

Opime Tablet interacts with simvastatin, which is used to lower the cholesterol levels. The combined use of multiple doses of 10 mg of Opime with 80 mg simvastatin may increase the level of simvastatin in blood. The dose of simvastatin should be reduced to 20 mg in patients who are on treatment with Opime.

Muscle Relaxant

There may be an interaction of Opime with dantrolene, which is used to treat muscle spasms. Use of dantrolene with Opime may see an increased risk of high potassium levels in the blood. Avoid use of this combination in patients with fever and severe muscle contractions.

CYP3A Inhibitors

Opime Tablet may interact with CYP3A4 inhibitors (diltiazem, ketoconazole, verapamil, itraconazole, erythromycin, clarithromycin, and ritonavir), which are used to treat high blood pressure, fungal, viral and bacterial infections. Use of CYP3A4 inhibitors may increase the level of Opime in blood. Clarithromycin, when used along with Opime, leads to an increased risk of low blood pressure. Careful monitoring and dose adjustment are required in patients using this combination.

mTOR Inhibitors

Your doctor's guidelines may need to be followed while taking this medicine along with mTOR inhibitors (temsirolimus, everolimus, sirolimus), which are used for treatment of cancer. Patients taking both mTOR inhibitors and Opime are at an increased risk of developing swelling in the lower layer of skin. Take necessary precautions before starting this therapy.

Immunosuppressant Drugs

Special instructions need to be followed while taking this medicine along with tacrolimus and cyclospoine, which are used to lower the risk of organ rejection. Use of Opime may lead to increased levels of tacrolimus and cyclosporine in the blood. Dose adjustment, as well as proper monitoring of levels of tacrolimus and cyclosporine in the blood, is required in patients who had undergone kidney transplantation procedure.

CYP3A4 Inducers

Opime Tablet may interact with CYP3A4 inducers (rifampicin, and hypericum perforatum), which are used to treat bacterial infections and skin wounds. When Opime is used in combination with CYP3A4 inducers may alter the level of Opime in the blood. Monitoring of blood pressure as well as dose adjustment is required in patients being treated with both the drugs.

When should Opime be not used?

Allergic to Opime, calcium channel blockers

This medicine should not be used in patients who are allergic to it. These patients may have the following symptoms if they use this medicine:
  • skin itching
  • reddening of skin
  • difficulty in breathing

Severe Low Blood Pressure

This medicine should not be used in patients with severe conditions of low blood pressure.

Insufficient Blood Flow to the Body

This medicine should not be used in patients with cardiogenic shock.

Heart Attack

This medicine should not be used during the first 28 days in patients suffered from the heart attack.

High Grade Narrowing of Heart Valves

This medicine should not be used in patients having high grade aortic stenosis.

Traveling With Medication

  • Ensure that you carry enough doses of each of your prescription medicines to last the entire trip. The best place to store your medicines is in the carry on baggage. However, while flying, if carrying liquid medicines, make sure you do not go over the limits imposed for carry-on liquids.
  • While traveling overseas, make sure that you can carry each of your prescription medicines legally to your destination country. One way to ensure this is by checking with your destination country's embassy or website.
  • Make sure that you carry each of your medicines in their original packaging, which should typically include your name and address, and the details of the prescribing doctor.
  • If your travel involves crossing time zones, and you are required to take your medicine as per a fixed schedule, make sure that you adjust for the change in time.

Expired Medication

Taking a single dose of expired Opime is unlikely to cause a side-effect. However, please discuss with your doctor or pharmacist, if you feel unwell or sick. An expired medicine may become ineffective in treating your prescribed conditions. To be on the safe side, it is important not to use an expired drug. Safe Disposal of Medication
